What are the stages of motor recovery?
- Flaccid paralysis
- Involuntary movement or flickers with external stim (tapping, stroking, associated rxn etc.)
- Able to move voluntarily in synergy, even components.
- Able to move in synergy and reverse if performed in weaker synergy first. Simple movements can be done out of synergy.
- Able to move in synergy and reverse regardless of which synergy in performed first. Relative independence from basic synergy.
- Near normal
- Normal
Describe the flexor and extensor patterns of the UE. Which is stronger?
Flexor pattern dominant in UE.
i) flex: SH retraction/ABD/ER/elevation, Elbow flex/sup, wrist flex, finger flex/ADD
ii) ext: SH Protraction/ADD/ER, elbow ext/pro, wrist ext, finger flex/ADD.
Describe the flexor and extensor patterns of the LE. Which is stronger?
Extensor pattern stronger in the LE.
i) ext: hip ext/ADD/IR, knee ext, ankle PF/inv, big toe ext, toes flex
ii) flex: hip flex/ABD/ER, knee flex, ankle DF/Inv, toes flex or ext
List ways to facilitate muscle recruitment in pts w/stage 2 motor recovery.
i) stroking/tapping: stretch reflex
ii) associated rxns: ex w/ other mvmt in synergy or yawn or contralateral mvmt
iii) brainstem rxns
What stage do you begin at for each body part when assessing CMSA?
Leg - stage 4
Arm, Hand, Foot - Stage 3
Pt may attempt each task TWICE only.
Stage 2 and 3 must be done in order presented.
