Assess pupils (size, shape, symmetry)
Visual neglect (cover one eye, how many fingers)
Visual fields (focus on my nose and tell me when you see my finger)
Pupillary reflexes (direct & consensual, swinging torch, accommodation reflex - distance then pen torch)
To further test - Ishihara plates for colour vision, Snellen chart for visual acuity and fundoscopy)
Test the CN3, 4 & 6?
Assess eye movements
H test (any double vision)
Test the CN 5
Trigeminal
Sensation on forehead, cheek and jaw - feel the same side to side
I would also use a neurotip
Clench jaw for me
Test for CN7?
Facial
Raise eyebrows
Close eyes
Blow out cheeks
Bare teeth
Test jaw jerk and corneal reflexes
Test for CN8?
Any changes in your hearing?
Whisper number in each ear
Rinne’s & Webers test
Test for CN 9 & 10?
Any changes in swallowing?
‘aahh’
cough
Test for CN11?
Shrug shoulders
Look to one side and don’t let me push you
Test for CN12?
Stick tongue out wiggle side to side
Push it into one side
Offer to do gag reflex
