Nervous system- Dementia Flashcards
What can be used to treat mild to moderate dementia?
acetylcholinesterase inhibitors
what can be used for dementia associated with parkinsons disease?
rivastigmine
When should the benefit of treatment be assessed?
coginitive assessment after 3 months
Name 3 anticholinesterases
donepezil, galantamine, rivastigmine
What type of inhibitor of acetylcholinesterase is donepezil?
reversible
When should donepezil be used with caution?
concomitant antipsychotic treatment. Increased risk of neuroleptic malignant syndrome
What are the side effects of donepezil?
aggression, hallucinations, pruritis, bradycardia
Why should you start donepezil at low doses?
causes dose related cholinergic effects
What type of inhibitor is galantamine?
reversible inhibitor and also has nicotinic receptor agonist properties
what type of inhibitor is rivastigmine?
reversible non-competitive inhibitor
How do you switch from oral rivastigmine to a patch?
apply patch on day following last dose. Transdermal less likely to cause GI disturbances
What should you do if you get GI disturbances on rivastigmine?
hold treatment until they resolve
What should you monitor with rivastigmine?
body weight
Name a NMDA (glutamate) antagonist?
memantine
When should memantine be used?
moderate to sever alzheimers
