What are the causes of nephritic syndrome?
A
Post-infectious GpA strep - 2w after URTI
IgA nephropathy (Berger’s) - days after URTI
Goodpastures’ syndrome
ANCA +ve vasculitis
Alport syndrome

How does it present?
A
Haematuria HTN Uraemia Oliguria Proteinuria (in smaller amounts than nephrotic)

What might you see in the urine?
A
Urinary casts
RBC casts
Urine will appear smokey in colour, rarely red

How is it IX?
A
HX
Measure BP
Look for oedema
Urine dip - Protein + blood
Bloods for underlying cause:
- FBC, U&E, LFT, ESR, CRP
- AAB - ANA, ANCA, dsDNA, anti-GBM
Imaging - CXR, renal USS
Renal biopsy**

How is it MX?
A
RX underlying condition
Fluid MX and salt restriction
Keep low BP, lower if >1g/d proteinuria
Furosemide
ACEi/ARB - decreases proteinuria and preserves renal function
Statin - hyperlipidaemia
Refer to nephrologist immediately
