Nephritic Syndrome Flashcards
Characteristics of Nephritic Syndrome (5)
Low GFR (vasoconstriction from inflamm) Urine Sediment (RBC, WBC, casts) Proteinuria (sub nephrotic) Edema (increased NA from low flow) HTN (increased NA from low flow)`
Primary Renal (6)
IgA Nephropathy Anti-GBM Membranoproliferative GN Vasculitis (ANCA) Hemolytic-Uremic Syndrome C3 disease
Systemic (6)
Henoch Schonlein Goodpasture Lupus Nephritis ANCA vasculitis Infection Ig deposition
Post Infectious GN
Group A Strep ICs deposit and activate complement Proliferative GN, inflammatory cells IF: granular IgG, C3 Check ASO for Dx Reversible damage
IgA Nephropathy
Mesangial Proliferation w/ IgA deposits w/ C3
Crescents uncommon
ESRD possible
Treat w/ steroids
MPGN
Mesangial proliferation and GBM thickening
Subendothelial/mesangial IgG deposits and C3
HCV associated
ESRD common
Anti-GBM
RPGN +/- pulmonary
ABs to collagen IV of GBM
Extensive crescenting
Early treatment w/ steroids and immunosups w/ plasma exchange
Pauci-Immune Renal Vasculitis
Small vessel vasculitis in kidneys w/o IC deposits
Crescenting
ANCA common
Lungs commonly involved
Treat: plasmapheresis, steroids, cyclophosphamide
Lupus
Renal issues common w/ lupus
Granular C3, C1q, IgG/M/A deposits throughout
Cryoglobulinemia
Common w/ HepC
IC deposits
Treat w/ anti-viral, rituximab (anti-B cell), and plasmapheresis
Henoch-Schonlein
Palpable Purpura, arthritis, GN, and GI
