What non-dietary factors should be considered as contributors to nutritional status?
Clinical factors, psychosocial factors, physical activity factors.
What comes under ‘clinical factors’ for data collection
Medical conditions, symptoms, treatments (pharmacological, surgical, radio therapeutic treatments)
Under psychosocial - what should be considered when doing a social assessment?
Living arrangements, food prep & storage, financial situation, literacy & numeracy, access to clean water, working conditions, educational & cultural background.
What psychosocial factors might impact nutritional status?
Psychotic illness, depression, anxiety, dementia, substance abuse, bereavement.
What factors should you consider when assessing PAL?
Stage of life, occupation, physical health issues/injuries, state of health (eg hospitalised), exercise (type, intensity, frequency, duration), flexibility.
What are the three main types of prediction equations to estimate energy expenditure?
1) Tables from nutrient reference values (3.1-3.4 in Griffith) + PAL
2) Energy requirements from body weight alone
3) Prediction equations based on BMR/REE and the factors which contribute to total daily EE (e.g. BMR x illness factor x activity factor)
What factors affect BMR?
Age, lean body mass, gender, starving/malnutrition, hormonal alterations, growth, fever, illness/metabolic stress/injury.
What are the equations for predicting BMR?
Schofield, Harris Benedict
What is the Schofield equation based on?
Age group, gender, body weight
How does a stress/injury factor affect energy requirements?
Increasing requirements due to adaptive thermogenesis. Example: a stress factor of 1.2 increases requirements by 20%.
Which disease states use specific equations to determine EER?
Cancer, ICU/ventilation, renal disease, liver disease.
