NCLEX review Flashcards
pH
7.35 - 7.45
CO2
35 - 45
HCO3
22 - 26
pH < 7.35
acidosis
pH > 7.45
alkalosis
ROME
respiratory
opposite
metabolic
equal
pH is low…
everything is low, EXCEPT potassium (high)
pH is high…
everything is high, EXCEPT potassium (low)
pH > 7.45 s/s
everything is up:
-tachycardia
-tachypnea
-HTN
-seizures
-irritability
-diarrhea
-increased bowel sounds
-hyperreflexia
ph < 7.35 s/s
everything is down :
-bradycardia
-bradypnea
-low BP
-absent bowel sounds
-constipation
-lethargy
-coma hyporeflexia
-flaccid
MAC Kussmaul
Metabolic ACidosis = Kussmaul respirations
underventilation leads to
respiratory acidosis
overventilation leads to
respiratory alkalosis
high pressure vent alarms
-kink in tubing
-condensed water in dependent tube
-mucus plug
-pneumothorax
-pt anxiety
-pt biting tube
low pressure vent alarms
-tubing disconnection
-cuff leak
-deflated cuff
-pt stops breathing / decreased breathing
too high of ventilation leads to…
respiratory alkalosis (think hyperventilation)
too little of ventilation leads to…
respiratory acidosis (retaining CO2)
ABG interpretation of ventilated pt shows respiratory acidosis…
they are NOT ready to be weened from the vent, they require higher settings
lithium
used for bipolar disorder
-therapeutic level = 0.6-1.2
-TOXIC level = >2.0
digoxin
used to treat a-fib and CHF
-therapeutic level = 1 - 2
-TOXIC = > 2
aminophylline
muscle relaxer for airway
-therapeutic level = 10 - 20
-TOXIC = > 20
-nontherapeutic level = < 10
phenytoin (dilantin)
seizure medication
-therapeutic = 10 - 20
-TOXIC = > 20
bilirubin adult level
0.2 - 1.2
bilirubin child level
10 - 20
TOXIC = > 20
**hospitalized at 14 - 15
kernicterus
excess bilirubin in the brain!!
- happens when level is > 20
-CAN BE DEADLY
**opisthotonic position will occur
pathological jaundice
newborn comes out with jaundice
**CONCERNING jaundice
physiological jaundice
newborn turns yellow 2-3 days after delivery
**this one is okay
thyroid gland
regulates metabolism
**change word to metabolism
hyperthyroidism
hyperMETABOLISM
-weight loss
-HR up, BP up
-they have heat intolerance… can tolerate cold
-exophthalmos : bulging eyes
what is also seen in hyperthyroidism that’s in Grave’s disease
exophthalmos!!!!
hyperthyroidism
- nuke it with radioactive iodine
- PTU - put thyroid under
- thyroidectomy - most common
what will total thyroidectomy need…
lifelong hormone replacement!!!
-at risk for hypocalcemia
hypocalcemia s/s
paresthesia
tetany
twitching
clonus
seizure
tremor
Chvostek and Trousseau sign
thyroid storm s/s
fever!!! > 105
high BP –> stroke level 210 / 180
severe tachycardia
delirium
thyroid storm interventions
- BRING temp DOWN : ice packs, cooling blanket
- O2 at 10L
hypothyroidism
hypoMETABOLISM
-obesity
-HR decrease, BP decrease
-personality flat, boring, dull
-cold intolerance!!
-academically challenged
myxedema coma
severe hypothyroidism
-decreased mental status
-hypothermia
-slowing of multiple organs
**DO NOT SEDATE these pts!!!
