Nasal ADME

A

It can be a form of either topical administration or systemic administration, as the drugs thus locally delivered can go on to have either purely local or systemic effects.

The nasal route of drug administration has several advantages over oral or intravenous administration, which include non-invasiveness, self-administration, shorter time to onset of effect and higher bioavailability due to avoidance of hepatic first-pass metabolism.

There are three principal factors that influence the extent to which drugs can be absorbed through the nasal lining (epithelium):

Physico-chemical characteristics of the drug molecule (Solubility)

The action of the mucociliary system within the nose

The presence of any factors increasing nasal absorption

In general, drugs can follow either extracellular or intracellular routes to gain access to the circulatory system or the nervous system

Extracellular transport across the columnar cells of the epithelium can occur via paracellular diffusion.

Why adventitious?

A

Tablets First Pass metabolism vs absorption in to subcutaneous tissue. Anti-Semitic was anti-sickness – so better route of administration based on patient symptoms.

You are making nasal spray with purified water, you have an OOS (400 cfu/mL) and you have been making the nasal spray for 3 months. What are your concerns? On questioning this is a single result with no trends. What are you expecting the Lab Investigation to cover? Got into sample handling.

Talked about LIs level 1a & 1b in lab, 2 plant investigation (including sample handling between the two), worked through 5 Ps (until they had enough), The 3 months appeared to be a red herring which I got a little stuck with but they let that pass. Led me through sample handling until it seems that it wasn’t refrigerated on receipt. I had asked about what organism and they turned that back to me for ‘what would I expect to see in a water system result’ so I went into examples for water systems and people (sample handling) but they didn’t tell me what it was.
