Nasal Obstruction Flashcards
1
Q
What are the surgical options for nasal obstruction?
A
- Address the area of obstruction
- Any components of the nasal valve - septum, turbinate, alar cartilage support
- Septoplasty
- Spreader, batten or overlay grafts
- Lateral crural or columellar strut grafts
- Nasal-orbital suspension
- Inferior turbinate reduction
- Flaring sutures
- Valvuloplasty with composite graft

Q
What are 6 methods for inferior turbinate reduction?
A
- Out-fracture lateralization
- Submucosal soft tissue reduction (electrocatuery, radiofrequency coblation, microdebrider)
- Resection of inferior turbinate bone
- Resection of inferior turbinate bone and lateral mucosa
- Partial mucosa soft tissue resection or ablation (ie. laser turbinectomy)
- Total turbinectomy

Q
Discuss middle turbinate removal
A
- No QOL or functional changes with resection
- Patients with inflammatory sinus disease actually smell better
- Likely from better topical delivery, no middle meatus obstruction, and better inflammation control
