Define myofascial pain dysfunction
non-articular TMJ disorder that is dull regional masticatory myalgia that worsens with function and can lead to decreased range of motion
most common TMJ disorder
4 etiologies of myofascial pain dysfunction
-parafunctional habits (bruxisim,nail biting,clenching,gum, chewing)
-life stressors
-apertognathia and/or overjet > 6 mm
-lack of posterior dentition leading to muscle hyperactivity
Patients w myofascial pain dysfunction complain of
pain on side of face
pain is diffuse and involves muscles of mastication
DDX myofascial pain dysfunction
Trigeminal neuralgia
Atypical facial pain
fibromyalgia
10 Treatments of myofascial pain
-NSAIDS to reduce pain and inflammation
-occlusal appliance
-soft diet
-muscle relaxants
-warm compress
-occlusal equilibration
-trigger point injections
-botox intramuscular injections
-replacing posterior dentition
-physical therapy
NSAID dosage for tx of myofascial pain
-ibuprofen
-naproxen
-mobic
-ibuprofen 600 mg QID x 2 weeks
-Naproxen 500 mg BID x 2 weeks
-Mobic 7.5-15 mg daily x 2 weeks
muscle relaxant dosages for tx of myofascial pain
-cyclobenzaprine
-baclofen
why do we prescribe them as QHS?
-cyclobenzaprine 5-10 mg daily QHS
-baclofen 5-10 mg TID
concern for dizziness/sedation
