Myocarditis Flashcards
1
Q
what is myocarditis?
A
inflammation of the myocardium
2
Q
causes of myocarditis
A
most commonly viral, Cox-sackie virus
- HIV
- diphtheria
- rheumatic fever
- radiation injury
- drug-induced
3
Q
clinical features of myocarditis
A
- acute illness
- fever
- biventricular failure
- cardiac arrhythmias
- pericarditis
4
Q
management of myocarditis
A
- bed rest
- treat like heart failure
5
Q
clinical findings on echo for myocarditis
A
- pericardial effusion
- hypokinesia
6
Q
types of myocarditis
A
- autoimmune myocarditis (either exclusive involvement of the heart eg Churg-Strauss syndrome or a systemic autoimmune condition like SLE)
- giant cell myocarditis
- drug-induced myocarditis
- sarcoidosis
- hypersensitive and toxic myocarditis
- eosinophilic myocarditis (rare but fatal; can be caused by a number of different reasons)
7
Q
how would you diagnose myocarditis?
A
tissue biopsy
8
Q
3 phases of myocarditis
A
- direct viral damage to the myocyte (the virus causing cell death)
- triggered immune system attacks the heart
- myocardium undergoes remodelling, causing the development of dilated cardiomyopathy (can cause HF)
9
Q
Characteristics of acute myocarditis
A
- lymphocytic infiltrates
- myocyte necrosis
10
Q
characteristics of borderlines necrosis
A
- inflammatory infiltrates
- no evidence of myocyte necrosis
