What is Myasthenia Gravis?

A

Autoimmune neuromuscular condition where antibodies (AChR-Ab) attacks the acetylcholine receptors on the skeletal muscles (end motor plate)

  1. ptosis (drooping eyelids)
  2. diplopia

Main symptoms of generalised MG?

A
  1. Facial - expressionless and Myasthenic sneer of the lips
  2. Bulbar - dysphagia, dysarthria, fatiguable chewing
  3. Neck - Dropped head syndrome
  4. Respiratory - during myasthenia crisis
  5. Limbs - weakness especially in arms and proximal muscles

Investigations for MG?

A
  1. Bedside: neuro exam, fundoscopy, Icepack test
  2. Bloods: FBC, haematinics, U&E, LFTs, bone profile, glucose/Hba1c, TFTs (Graves)
  3. Special test: serological testing (AChR-Ab), Repetitive Nerve Stimulation test
  4. Thymus assessment (CT)
  5. Rheumatological tests: ANA, CCP, RF (RF, SLE)

A

Put ice on the eye with ptsosis and it should provide temporary relief
