Apley Scratch Test:
- Hugging themselves
- Reaching up
- Reaching down spine
- Internal rotation and aDduction
- External rotation and aBduction

* If unable to perform AST, suspect rotator cuff involvement (subscapularis)
Neer’s Test
Impingement
Positive = pain
Hawkin’s- Kennedy
Impingement
Positive= pain
Lift Off
Rotator cuff injury, likely subscapularis
Positive = pain/inability
Drop Arm Test
Rotator cuff tear
Positive= inability to hold arm/control adduction below 90º
Apprehension Test
Shoulder dislocation/subluxation or instability
Positive = visible apprehension from patient’s reaction
Yergeson’s Test:
Bicep tendinopathy or subluxation of biceps tendon
Positive = pain
Speed’s Test
Bicep tendinopathy
Positive = pain
Empty Can Test
Rotator cuff/supraspinatus
Positive = pain
Valgus Stress Test
MCL of knee or ulnar collateral ligament of elbow
Press medially
Varus Stress Test
LCL of knee or RCL of elbow
Press laterally
Cozen’s Test
Resisted wrist extension
Positive = pain
3rd Finger Test
Stresses extensor digitorum and extensor carpi radialis brevis
Positive = pain
de Quervain’s Tensosynovitis
Swelling or stenosis of the sheath that surrounds the abductor pollicus longus and extensor pollicus brevis.
Symptoms include pain associated with pinching, gripping, wrist or thumb movements
Finkelstein’s Test
Fingers flexed around thumb and examiner twists wrist into ulnar deviation
Positive= pain reproduced over the radial wrist
Faber Test
Determines if pain is from hip or SI joint
If pain is lateral or inguinal– hip issue
Posterior pain– SI joint
Thomas Test
Tests for tight hip flexors
Positive = opposite side leg rises meaning tight hip flexors on the side of the leg that is rising
Ober Test
Evaluates IT band tightness
Positive if knee/leg does not move back down towards table
Ballottement Test
Evaluates for effusion inside knee joint.
Indicates intra-articular injury
Positive = feels like patella is floating
Apley’s Compression Test
Patient is prone with knee bent. Physician compresses joint and twists.
Tests for meniscal tear
Apley’s Distraction Test
Tests collateral ligaments
Stabilize prone thigh on table. Lift lower leg and twist
McMurray’s Test
Tests medial and lateral menisci
Apply external rotation to tibia and VALGUS maneuver to test medial meniscus
Apply internal rotation to tibia and VARUS maneuver to test lateral meniscus
Anterior drawer test of ankle tests which ligament
Anterior talofibular ligament (ATF)
Talar Tilt tests which ligaments (2)
Lateral instability of ATF and CF (calcaneofibular ligament)
