Origin: Medial superior nuchal line and external protuberance of occipital bone, ligamentum nuchae, and spinous process of C7-T12

Upper Fibers - elevate and upwardly rotate scapula; extend neck
Middle Fibers - adduct (retract) scapula
Lower Fibers - depress and help upper fibers upwardly rotate scapula

Insertion: Superior aspect of greater tubercle of humerus

Insertion: Medial lip of intertubercular (bicipital) groove of the anterior humerus

Nerve: Lower Subscapular Nerve (C5-C7)
