Baclofen - use
spasticity in MS and some spinal cord injury
doesn’t decrease muscle strength
Baclofen - AE
drowsiness, dizziness, weakness and fatigue
N/V, constipation, and urinary retention (low risk)
can exacerbate psych
- overdose - resp depression
- withdraw slowly
Baclofen - contraindications
- alcohol and CMS depressants
- h/o urinary retentions, BPH, or taking a drug that causes urinary retention
diazepam (valium) - use and class
benzodiazepine
GAD, seizures, muscle spasm and spasticity, Alcohol withdraw, and preanesthetic
diazepam (valium) - AE
sedation
dantrolene - use
acts on skeletal muscle
spasticity associated with MS, CP, and spinal cord injury
- may see decrease in strength
- if no benefits in 45 days, d/c
dantrolene - AE
hepatic toxicity -most common in women > 35 BBW
muscle weakness, drowsiness, diarrhea, N/V, dysphagia, hoarseness, and ED
cyclobenzaprine - uses
1st choice for muscle spasm r/t epilepsy, hypocalcemia, and injury
cyclobenzaprine - AE
CNS depressants (decrease with continued use), anticholinergic effects, cardiac dysrhythmias
cyclobenzaprine - contrindications
MOA inhibitors SSRI and SNRI, pts with cardiac rhythm abnormalities
Valium - contraindication
glaucoma, respiratory or liver impairment, OSA, and Myasthenia gravis
Metaxalone - contraindications
impaired hepatic or renal impairment, h/o anemia, increases r/o serotonin syndrome in patients taking SSRI’s
methocarbamol - contraindications
avoid in patient with seizures or renal impairment; hepatic impairment
orphenadrine - contraindications
glaucoma, myasthenia gravis, GI obstruction, bladder obstruction, heart disease or failure
