What muscle disease is associated with temporal arteritis?
polymyalgia rheumatica
How does polymyalgia rheumatica present?
proximal myalgia (shoulder and hip)
improves as the day goes on and with movement
morning stiffness >1 hour
decreased range of movement
How is polymyalgia rheumatica diagnosed?
increased inflammatory markers
dramatic response to low dose steroids
How is polymyalgia rheumatic managed?
prednisolone
dose gradually decreased over a few months
How does polymyositis present?
insidious onset
symmetrical proximal muscle weakness
noticed when performing an activity e.g. stairs, brushing hair
organ involvement - dysphagia, ILD
How is polymyositis diagnosed?
increased CK, inflammatory markers
muscle biopsy
Anti-Jo-1 antibodies
How is polymyositis managed?
prednisolone
immunosuppression (methotrexate, azathioprine)
How does dermatomyositis present?
clinically similar to polymyositis + cutaneous features
What are the cutaneous features of dermatomyositis?
V shaped rash over chest
photosensitivity
heliotrope rash in periorbital region
gottron’s papules
What are the antibodies for polymyositis?
anti-jo-1
What is dermatomyositis associated with?
ANA
What is fibromyalgia?
widespread pain throughout body with tender points at specific anatomical sites
How does fibromyalgia present?
chronic pain, all over, lethargy, cognitive impairment, sleep disturbance
How is dermatomyositis diagnosed and managed?
same as polymyositis (but ANA)
