What is a PREM1 form used for?
used by community pharmacies to notify NHS England before providing MURs on premises
What is a PREM2 form used for?
used to make applications to local NHS England team for consent to undertake MURs off premises
What is a PREM2a form used for?
alternative premises with a consultation area (e.g. GP practice)
What is a PREM2b form used for?
alternative premises for a particular patient and occasion (e.g. patient’s home)
What is a PREM2c form used for?
alternative premises or category of premises for a category of patients (e.g. care home)
What is a PREM2d form used for?
MUR by phone for a particular patient and occasion (no one must be able to overhear the conversation)
MURs can only be provided for patients who have been using the pharmacy for dispensing of their prescriptions for the previous how many months?
3 months
Can an MUR be conducted with a child?
If the child is competent and able to fully engage in discussion with the pharmacist.
What are the requirements for an MUR consultation area?
The patient and the pharmacist can sit down together;
They can talk at normal speaking volumes without being overheard by staff or customers; and
The area is clearly signed as a private consultation area.
What are the 4 national target groups for MURs?
Patients taking high risk medicines
Patients recently discharged from hospital who had changes made to their medicines while they were in hospital.
Ideally patients discharged from hospital will receive an MUR within four weeks of discharge but in certain circumstances the MUR can take place within eight weeks of discharge
Patients with respiratory disease; and
patients at risk of or diagnosed with cardiovascular disease and regularly being prescribed at least four medicines
From 1st April 2015, community pharmacies must carry out at least how many of their MURs on patients in one or more of the target groups?
