DDx
tricuspid regurgitation but no vwaves and no pulsatile liver
VSD in a young patient

Degenerative
IHD and LV dysfunction esp if poor pulses
Rheumatic
MVP
Severe
low volume slow rising pulse
narrow pulse pressure
Thrill
Heaving apex, undisplaced
Harsh ESM
