What is the purpose of the multiaxial assessment?
The purpose is to ensure attention is given to certain types of disorders, aspects of environment, and areas of function that might be overlooked if the focus were on assessing just one problem.
What is reported on Axis II?
Personality disorders and mental retardation.
What is reported on Axis III?
General Medical Conditions.
What is reported on Axis IV?
Psychosocial and environmental problems that affect the diagnosis or care of patient.
What is reported on Axis V?
GAF scale.
Removal from the home.
Problems with primary support group.
Living alone.
Problems with social environment.
Illiteracy.
Problems with education.
Threat of job loss.
Problems with occupation.
Unsafe neighborhood.
Problems with housing.
Extreme poverty, inadequate finances.
Problems with economic.
Arrest, incarceration, victim of crime.
Problems with interaction with the legal system/crime.
Exposure to disasters, war, other hostilities.
Problems with other psychosocial and environmental problems.
Some mild symptoms OR some difficulty in social, occupational, or school functioning, but generally functioning pretty well, has some meaningful interpersonal relationships.
