Causes of shoulder impingement syndrome

A

Any condition that limits space within coracoacromial arch:

  1. Abnormal acromion shape
    • type III, inf hook
  2. Abnormal acromion orientation
    • ant downslope (sag)
    • inferolat tilt (cor)
    • low-lying
  3. AC degen
  4. Os acromiale
  5. Thick CoAc ligament
  6. p-traumatic osseous deformity
  7. instability
  8. Muscle hypertrophy

Def: interrupted sheath around LHBT with intact rotator cuff

Type 1- isolated pulley tear (CHL-SGHL sling)
Type 2- Type 1 + articular-sided tearing of SUPRA + mild medial subluxation LHBT
Type 3- Type 1 + articular-sided tearing of SUBSCAP + mild medial subluxation LHBT
Type 4- Type 1 + articular-sided tearing of SUPRA & SUBSCAP + DISLOCATED LHBT
