What is the general appearance and behavior prompt?
Im assessing general appearance, motor behavior and attitude. I see that the patient appears to be well groomed, with appropriate hygiene and clean, casual clothing.
Posture is upright and eye contact is being maintained throughout the interaction. Facial expressions are appropriate, and there are no signs of distress or discomfort.
Motor behavior is normal, with no evidence of tremors, fidgeting, or psychomotor agitation. The patient has been cooperative and engaged during the assessment.
Visual and Auditory Hallucinations?
Have you seen or heard anything unusual that others do not seem to notice?
Pt denies experiencing auditory or visual hallucinations. There are no signs of the patient responding to stimuli upon observation.

Have you had any unsuual sensations on your skin, noticed strange smells that others dont, or experienced odd tastes without explanation?
No tactile, olfactory, or gustatory hallucinations are reported or observed.
Depersonalization and Derealization
Have you ever felt disconnected from your self like you were watching yourself from outside of your body?
Have you ever felt like the world around you was unreal, like a dream?
There are no signs of depersonalization or derealization.
