relapsing-remitting
most common, months to yrs, learning disability, visual loss 1 eye, numb/tingling. re-myelination occurs between bouts. oligodendrocytes heal but die over time dt damage is irreversible
secondary progressive
Immune attach is constant, attacks last longer happens after relapse remitt, and cont. to get worse, no wax or wane anymore
primary progressive
constant attack, progressive disability, pain, HA, Lhermitts sign, paralysis, numbness, diff walking
progressive relapsing
normal ms s&s
S&S
weak, numb/tingle, unsteady gait, spastic, paraparesis, optic neuritis, sexual dysfunction, tremor, nystagmus, diff speaking , eating,swallowing. worse after hot shower, diff thinking, poor concentration
charcots neurologic triad
dysarthria, nystagmus, intentional tremor
lhermitte sign
electric shock runs down back and radiates to limb when bending neck forward
test
mri is gold standard… sees lesions/plaques, once diag McDonalds criteria is used to evaluate
LP: csf has high levels of antibodies and IgG
visual evoked potential: measures nervous system resp to stimuli
tx
relapse-remit: corticosteroids, IV IgG, cyclophosphamide help to decrease freq of relapse,
plasmapheresis removes antibodies
chronic tx: immunosuppressants block t cells from getting into brain
manage other symptoms: depression, PT , cognitive rehab, it D,
