Unintentional - patients want to comply but forgot dosage instructions or misunderstand directions
Intentional - discontinuation due to side effects

Proportion of a drug which enters circulation and is available to have an effect

Conc of a drug is reduced before it reaches systemic circulation due to it being reabsorbed by the liver
