Origin - humeral head - medial epicondyle (i.e. via common flexor tendon). Ulnar head - posterior border of ulna; olecranon.
Insertion - pisiform, hook of hamate (via pisohamate ligament), base of the 5th metacarpal (via the pisometacarpal ligament).

Origin - Medial epicondyle of the humerus
Insertion - Base of the 2nd and 3rd metacarpal bones
