Pathogenesis of Campylobacter
Infective dose:10^4 organism
Organism multiplies in the small intestine.
Invades the epithelium producing inflammation.
Rbc wbc present in stool ( bloody diarrheoa)
Occasionally blood stream is invaded and systemic infection occurs.
Clinical features of Campylobacter
1.Acute onset of abdominal pain.
2. First watery foul smelling diarrheoa followed by bloody diarrheoa
3. Fever
4. Malaise
5. Systemic infections occur mostly in neonates and immunocompromised person
6. Certain serotypes are associated with post diarrheoal guillian barre syndrome
7. Infection is also associated with autoimmune diseases like reactive arthritis and reiters syndrome.
Guillian barre syndrome
It is an autoimmune disease
Attributed yo the formation of ab against c. Jejuni that cross reacts with ag of neuron resulting in ascending paralysis.
Lab diagnosis of Campylobacter
Specimen: stool
Smear: gram stained smears of stool show typical darting motility of the organism
Culture : skirrows media which is selective media. It contains
Vancomycin
Trimethoprim
Cephalothin
Polymixin
Amphoterecin B
