What is hypomania?

A

A lesser form of mania that lasts several days

What are the symptoms/signs of a hypomanic episode?

A
  • Mildly elevated mood
  • Increased energy & sense of wellbeing
  • Sociability, overfamiliarity, sexual energy & decreased sleep
  • Reduced conc
  • New interests & mild overspending
  • Irritability

Hypomania shouldn’t cause severe disruption of work or social rejection

What are the symptoms & signs of mania?

A

Same as hypomania but worse +:
- Disinhibition
- Grandiosity
- Altered senses
- Extravagant spending
- Frequent irritability
- Pressure of speech
- Flight of Ideas

This tends to last ~1wk and is severe enough to completely disrupt work and socialising

A

Drugs & alcohol
SLE
Stroke, MS, tumour, epilepsy, AIDS & neurosyphilis
Cushing’s or Hyperthyroid

Mixed Affective state
Schizo(phrenia)(affective disorder)
Cyclothymia
ADHD

What tools do we use to assess mania?

A

SCID & SCAN

YMRS - Young Mania Rating Scale (mostly for monitoring how symptoms change)

How can we treat Mania?

A
  • Antipsychotics
  • Mood Stabilizers (anticonvulsants & lithium)
  • ECT

What is BPD?

A

Bipolar disorder is a swinging of mood between depressive episodes and manic episodes

How do we diagnose BPD?

A

2+ episodes of depression & (hypo)mania

Mania is BPD, it cannot be diagnosed as just Mania.

When does BPD/mania tend to onset?

A

Earlier than depression, avg at 21.
Early onset i.e. <20 is commoner with a FH
