What are the 3 cardinal signs of depression?
- Depressed mood.
- Loss of interest and enjoyment (anhedonia).
- Reduced energy levels, easy fatigability
Give 5 associated symptoms of unipolar depression.
- Reduced concentration & attention
- Reduced self-esteem & self-confidence
- Ideas of guilt & worthlessness
- Bleak & pessimistic views of life
- Ideas or acts of self-harm or suicide
- Disturbed sleep
- Change in appetite (⁄) with corresponding weight change).
- Change in psychomotor activity, with agitation or retardation (either subjective or objective)
How long do depressive symptoms need to persist before a diagnosis can be made?
2 weeks
What has to be excluded from the history to make a diagnosis of unipolar depression?
No history of mania or manic episodes
To diagnose somatic depressive syndrome, 4 of 8 symptoms are required. What are these 8 symptoms?
- Anhedonia
- Lack of emotional reactivity to normally pleasurable surroundings.
- Early wakening (2 or more hours before usual time).
- Depression worse in the mornings. (diurnal variation)
- Objective evidence of psychomotor retardation or agitation (remarked on or
- reported by other people).
- Poor appetite & weight loss (5% of body weight in the past month).
- Loss of libido
What are the diagnostic criteria for:
- mild depression?
- moderate depression?
- severe depression?
- severe depression with psychosis?
Mild –with/without somatic symptoms
- 2 of cardinal symptoms plus two others (2+2)
- Usually able to continue ordinary work and social activities
Moderate symptoms–with/without psychotic
- Two of cardinal symptoms plus three/four other symptoms (2+4)
- Considerable difficulty in continuing with social, work or domestic activities
Severe – without psychotic symptoms
- All three cardinal symptoms plus at least five other symptoms(3+5)
- Unlikely to continue with social, work or domestic activities
Severe , with psychotic symptoms
- • Above criteria , plus
- • Mood congruent/incongruent psychotic symptom: delusions, hallucinations or depressive stupor
