what is a mood disorder?

A

disturbance in mood as primary or predominant feature; loss of control over their mood and loss of control with distress

Major depressive disorder presentation

A

5 in a 2 week period including DEPRESSED MOOD/ANHEDONIA
depressed mood, fatigue, loss of energy, loss of pleasure of interest (anhedonia), increase sense of worthlessness and guilt, significant weight loss or gain, decrease concentration, insomnia or hypersomnia, morbid thoughts w/ suicidal ideation, agitation or retardation

ETIOLOGY of Major Depressive Disorder

A

Biological Factors: serotonin and norepinephrine disregulation, sleep abnormalities
genetics
psychosocial
as pt. ages, they tend to have more frequent episodes that last longer

Dysthmia

A

nonpsychotic symptoms of depression that do no meet major depression criteria. twice as common in women than men. For at least 2 years
2+: poor appetite, overeating, insomnia or hyposomnia, low energy and fatigue, low self-esteem, poor concentration or difficulty making decisions, feelings of hopelessness
The person has never been w/out these symptoms for more than 2 mnths.

Antidepressant medications: Tricyclics

A

common: amytiptaline (Elavil), nortryptyline (Pamelor), imipramine Tofranil, Desipramine (Norpramin)
lots of side effects
seratonin and norepinephrine

Antidepressants: SSRIs

A

much safer in overdose than in tricyclics and MAOIs
most widely prescribed
side effects: GI probs, decreased appetite, insomnia, sexual dysfunction, restlessness/agitation, “serotonin syndrome”-when combine w/ MAOIs

Antidepressants: MAOIs

A

rising number of neurotransmitters in the brain by inhibiting monmine oxidase enzyme
Dietary restrictions: cannot have tyramine it will elevate BP, tyramine low diet could have hypertensive crisis w/ certain foods
