If just one joint affected what thing would you be thinking?
osteoarthirits-could just be in one joint, maybe there was an injury
psoriatic arthritis ( early sign)-gradual
Septic arthritis- come on quickly, red, swollen
gout
reactive arthritis
if polyarthiritis?
rheumatoid arthritis
OA- (as some patients wait until quite a few joints affected before they come)
What is important when taking a rheumatology history?
Is there evidence of inflammatory joint/muscle condition?
Are systems outwit the MSK system affected?
How do the symptoms impact upon the patient?
What would suggest the mucocutaneous system is affected?
Alopecia, oral or nasal ulceration, nosebleeds, malaria rash,photosensitivive rash, dry mouth, tightening or thickening of the skin of the fingers that suggest sclerodactyly
How can you determine if the chest is involved?
Pleuritic chest pain
SOB
Cough
haemoptysis
What PMHx is important to know?
asthma., peptic ulcer ect as NSAIDs would not suit them
What are bony swellings of DIP joints
Heberden’s nodes
What are bony swellings of the PIP joints
Bouchard nodes (remember because further back on the hand)
