What is Fibromyalgia? *
Chronic condition that causes widespread pain, sleep problems, fatigue, mental and emotional distress
- Abnormal pain processing
- Cannot be cured!! but can be managed
Risk Factors:
- Age
- Co-existing lupus or RA
- Female gender
- Stress or traumatic events
- Repetitive injuries
- Illness
- Family history
- Obesity
What Causes Fibromyalgia?*
CNS Disorder
- Central sensitization

- Autoimmune disease
- Inflammation
- Viral Syndrome
What Non Drug Measures are Recommended? *
- Aerobic exercise (20-30min 2-3xweek) !!!! and should be used concurrently with any pharm
- Resistance training
- CBT
- Meditative movements such as tai chi or yoga
- Hydrotherapy
- Mindfulness
- Acupuncture
If Non Drug Fails, What Pharm can be Used? *
SNRI’S
Duloxetine
- 1 in 6 achieve 30% reduction
- FDA approved for FM
Milnacipran
- 1 in 6 pts achieve 30% reduction
- FDA approved for FM
Amitriptyline
- 1 in 4 achieve 30% reduction
- FDA approved
- possible anticholinergic side effects
Pregabalin
- FDA approved
What is the role of analgesics for FM pain?*
- NSAIDS: not ideal
- Capsaicin: not ideal
- Opioids: dont
- also dont with benzos, and sleep meds
