Cold Pack Precautions/Contraindications

A

-Cold hypersensitivity (vascular skin reaction, decrease in bp, syncope, severe itching
-Cold Intolerance (pain, numbness, color changes)
-Cryoglobulinema (abnormal proteins in blood, blood in urine)
-Raynauds (vasoconstriction of cold in fingers/toes)
-Regenerating neve
-Circulatory compromise
-Frostbite

-Grab tongs and obtain hot pack
-Place on Terry cloth and wrap

-Set clients arm on towel
-Turn on ultrasound
-Click Ultrasound > Clinical procedure > Acute > Superficial > 1 mHZ (5cm); 3mHZ (2-3cm) > 8 min
-Squeeze gel on affected area
-Take head and place in gel in contact with skin
-Use other hand to toggle knob to read 5 watts per cm2
-Massage in gel until beeps, do not remove nozzle until beeping noise
-Set nozzle in holder and wipe clients arm with towel
-Sterilize nozzle with alcohol pad
-Check skin

-Turn on ultrasound
-Click “ultrasound” > “clinical procedure” > Chronic > Superficial > 1 MHZ > 8 minutes
-Squeeze ample gel on affected area
-Take ultrasound head, insert head in gel and in contact with the skin
-With other free hand, toggle knob to read 1.5 watts per cm2

Example SOAP Note

A

S: Client reports pain at 4/10. “I can’t reach overhead like I need to.” Client post treatment reports pain at 2/10
O: Moist hot pack applied to shoulder with patient seated and arm resting on table for 15 minutes with 6 layers. Moist hot pack followed by manual stretching and education of wall slides as part of HEP. Skin intact pre and post treatment.
A: Client tolerated the treatment session well and was able to demonstrate understanding of HEP after instruction. Client pain levels improved by the end of session and he was able to tolerate HEP instruction without increased discomfort.
P: Upgrade HEP to include scapular stabilization and overhead reach ergonomics.
