What does a level V investigator do?

A

As a Level V investigator I handle cases where the claims range from $1-5 million. Level V investigators only deal with claims up to $5 million, CBB does not account for double indemnities in assigning cases so this was not out of the ordinary.

What is a double indemnity claim?

A

A double indemnity clause is a where the insurance company agrees to double the claim if death occurs by murder or accident

What happens if the death is not by murder or accident, and is instead the result of suicide?

A

If the death is a suicide or of natural causes then there is no payout , because it is not good public policy to be able to profit by taking your own life.

A

I was assigned to investigate because the insurance claim was for $5 million and had a double indemnity clause.

How did you investigate this case?

A

I did what I normally do. I looked through the files and I arranged to talk with the claimants and any witnesses that could shed light on the case.

What, if anything, did you find in the case files?

A

I examined claims from both Isabella, his widow, and from his child Lee/Leah a month after Isabella’s. (put in the dates). I found a copy of his insurance application, the insurance policy, the death certificate and the claim form submitted by Isabella Hizeneye.

A

She seemed upset- she spoke very fast and tried to avoid speaking about the death. She appeared very nervous, as if she was trying to hide something

What if anything did you learn from interviewing Isabella?

A

I learned Isabella was concerned that I might think that she harmed her husband.

A

I told her I didn’t believe she harmed her husband because she was not the beneficiary and therefore had nothing to gain

You said earlier you talked to Lee, What did you learn from Lee?

A

I learned that her/his dad had been acting strangely, sometimes crying, seemed exhausted and had not been sleeping much prior to the death.

Did you do anything else as a result of your investigation?

A

Yes, I examined a promissory note and a letter from Axon Bank and Trust which indicated Liam’s company was over $5 million in debt and did not have the means to pay it off. I made a note in my file that it was suspicious that Liam died just a few months after buying a 5 million dollar insurance policy while $5 million in debt. I also noted that if this claim was allowed, Liam’s family would be free of his debt and also receive an additional $5 million

A

I was asked to leave the house after discovering the documents. However I returned the next day.

Why did you return?

A

I had a suspicion I was missing something and hoped to confront Isabella about Liam’s death once and for all.

Did you end up confronting Isabella?

A

Yes. When I saw Isabella I prevented her from leaving by telling to her that I’d called the police on her. She then went to the office showed me a suicide note which she claimed was Liam’s.

Did the suicide note impact your deductions from the case?

A

Based off of the suicide note, I believe that Isabella’s seemingly guilty conscience was a result of her hiding the note rather than, her intentionally killing Liam

A few months before his death

A

Their death must be determined to be an accident or murder- this excludes suicide and natural causes
