mixed dysarthria Flashcards
define mixed dysarthria
occurs when neurologic damage extends into two or more parts of motor system
what is mixed dysarthria characterized
combination of characteristics in single pure dysarthria
causes of mixed dysarthria
various disorders affecting two or more parts of motor system
Multiple Sclerosis
progressive demyelinating disease
where can MS occur
in brainstem, cerebellum, cerebral hemispheres, and spinal cord
what can MS cause
any pure or combination of mixed dysarthria
what mixed dysarthria’s are most common in MS
ataxic-spastic
multisystems atrophy
collective term for group of degenerative disorders (many including parkinsonian symptoms: shy-drager syndrome, progressive supranuclear palsy, olivopontocerebellar atrophy)
ALS
progressive degeneration of any of four areas of motor neurons
does ALS often progress from affecting two motor neuron groups to all four?
yes
wilson’s disease
very rare hereditary preventing normal metabolism of dietary copper
what dyarthiras present in wilson’s disease
ataxic-spastic-hypokinetic
ALS mixed dyarthria’s
spastic-flaccid
friedreich’s ataxia
rare, inherited, progressive disorder
what does friedreich’s ataxia cause
neuron degeneration in cerebellum, brainstem, and spinal cord
what type of dysarthria is most common in Friedreich’s ataxia
ataxic-spastic
general rule for tx of mixed dysarthria
first treat component most severely affecting speech production
what is the order of tx of speech systems
respiration, resonation, phonation, articulation, prosody
caveats of treatment in mixed dysarthria
- multiple problems in single component of speech: treat most severe first
- patient may have strong preference for what needs to be treated initially
- patient’s attention or memory deficit may make working on one deficit too difficult
