Name (not ‘brother’) and address of supplied
Prescriber name, ID no and authorising document (FP10)
Amount supplied
Collecting person (& address if healthcare professional)
Was ID requested (can write ‘patient known’)
Was ID shown (if not why not, if yes what?)

You can record addict all info if you wish.

Why do we not enter CDs in the POM book

A

CD reg already keeps legal record.

However good practice MEP says also use POM book and cross reference no. With CD reg.

A

Who has access (i.e. method of achieving an auditable trail)
Where stored
Security in relation to storage and transport
Disposal & destruction
Who to alert if problem arises e.g. stock balance wrong
Record keeping re. register maintenance and Sch 2 returns by patients e.g. no longer needed

No - cannot reuse.
But you must record receipt in separate book or folder.
Record destruction in this folder with a witness.

A

Eg. Out of date.

Need a witness, record in CD register.

Who can get a licence to destroy CDs?

A

Pharmacies do not require one.
GPs might apply for one.
Ships, rigs cannot destroy CDs but return them to pharmacy for destruction.

Can you reuse medicines?

A

It is legal according to the home office but the RPS and MHRA do not permit it - bad practice.
SO DON’T DO IT.

You don’t know where it has been stored for example, right temperature/humidity?

Do you need to keep a CD reg for Sativex?

A

No. But you can anyway to maintain an audit trail.

Who can prescribe Sativex?

A

Only a doctor. No non-medical prescribers.
