ARDS - Acute respiratory distress syndrome

A

Respiratory failure characterized by widespread inflammation in the lungs and alveolar collapse.
Ventilator settings:
- Pressure mode
- High PEEP: 12 - 14 to recruit alveloi
- Low VT to reduce barotrauma: 6 - 8 ml/kg of Ideal Body Weight (IBW)

COPD - chronic obstructive pulmonary disorder

A

COPD pts tend to air trap and have long expiratory phases. Elevated CO2 may be normal.
Ventilator settings:
- RR: do not increase too much because of CO2 readings, as this may increase air trapping by decreasing exhalation time
- Flow rate may need to be increased to 80 - 100L/min (60L/min normal) to allow for longer expiration
- Keep IFR high and administer bronchodilators
