What medications are used in prophylaxis of migraines?
Propranolol
Amitriptyline
Topiramate
MOA of topiramate
Inhibits glutamate kainate and AMPA receptors
Also enhances GABA
MOA of propranolol
Vasodilates vessels in brain to enhance blood flow
MOA of amitriptyline
Inhibits the reuptake of noradrenaline and serotonin at respective nerve terminals
What medications are used in acute presentations of migraines?
Triptans e.g. Rizatriptan
When is prophylactic medication indicated?
If the migraines severely impair day to day life or are frequent (once a week)
How are migraines described?
Throbbing
Are the unilateral or bilateral?
Bilateral in 60% of cases
What worses migraines?
Physical activity
Epidemiology
Female>male
Headache duration
4-72 hours
Recurrent episodes
What makes it worse?
Aggravated by routine activities
Associated symptoms?
Nausea
Photophobia
Phonophobia
VIsual aura
Scotomoa - zig zag lines
Sensory aura
Pins and needles
What other auras are there?
Speech language symptoms
Atypical weird things like hemiparesis
What meds?
Ibuprofen/aspirin/paracetamol
Triptans
Antiemetic
What would you advise?
Acute medication should be taken early whilst pain is mild
What might trigger a migraine?
Tiredness/stress Alcohol Menstruation Bright lights Food - see next slide
Explain the relationship between food and migraines and what you should avoid
Lack of food or dehydration
Foods e.g. cheese, chocolate, red wines, citrus
