posterior- poor prog in most cases
whats a special anterior fdp case? whats its characteristics
anterior cases are generally “special”? but especially; maxillary lateral incisor pontic, with canine abutment
- solid abutment but delicate pontic
- rotation is prevented by tongue and lips
- very GOOD prognosis!
what is a Pier Abutment?
abutment tooth with both adjacent teeth missing. could be single piece or two piece FDP
what is a single piece pier abutment? and its characteristics
a pier abutment type of fdp;
- doesnt violate antes law
- pier becomes fulcrum ?
- downward forces in posterior create unseating forces in anterior (of the denture) this causes frequent de-cementation
what is a two piece FDP and its characteristics?
- anterior segment cemented first (pier is part of it?)
- posterior segment cemented to abutment , resting in distal of pier
- pier no longer acts as fulcrum ! (why?)
- metal frameworks only !!!
(non rigid, stress relieving option..)
what determines abutment teeth suitability?
- periodontal siupport; length of roots, bone loss, crown to root ratio
- number of roots/divergence of roots (configuration)
- endodontic status (vitality test, apical lesions, adequacy of fill, over-instrumentation (loss of radicular dentin to files?), extent of preparation, appropriateness of posts
- root ORIENTATION (?)
- teeth being replaced (Antes Law)
what is Antes Law?
The peri-cemental surface area of the abutment teeth to be used for a fixed dental prosthesis must be equal to or exceed the peri-cemental surface area of the teeth being replaced
(ex; cant use 18 and 21 abutments for missing 19 and 20, but can use 18, 21 AND 22 as abutments for it!)
what occurs if you violate antes law?
- torque on abutments
- fracture of porcelain veneers
- difficulty with retention and draw
- de-cementation
- inaccuracies in impressions
what axis should path of draw be on
parallel to long axis of the tooth (make a parallel long axis, they dont actually have )
-average of the two long axis- this is the path of draw.
