What is otitis media with effusion also known as?
Glue ear
What is otitis media with effusion
Inflammation of the middle ear which can present with STERILE fluid in the ear.
Due to dysfunction of the Eustacian tube- it creates a vacuum which drains out fluid from the inner ear
How to manage otitis media with effusion
- Observation for 3 months
- If no improvement then grommets/otovent
What are the symptoms of otitis media with effusion?
- Hearing loss
- Speech delay
What is acute suppurative otitis media?
- Build up of malignant fluid in the ear.
- Usually related to a burst ear drum- pain will increase until the eardrum bursts then will die down
- Will usually present with a history of URTI
What are the symptoms of acute suppurative otitis media?
- otalgia
- otorrhoea
- Bulging ear-drum
Management of acute suppurative otitis media?
- Observation
- Amoxycyllin
Tympanosclerosis
Calcification of the tympanic membrane and/or middle ear
- No management
- No presentation
What is chronic suppurative otitis media and what is it due to?
Chronic discharge of malignant fluid from the ear.
Aetiology:
-Choleastoma
-Perforated tympanic membrane
Consequences of suppurative otitis media?
Fluid can make its way into the inner ear
- Dead ear
- Facial palsy
- Meningitis
- Brain abcess
What is Choleastoma?
Abnormal growth of Skin in the middle ear
Usually forms a pouch that doesn’t shed so builds up.
It perforates into important structures such as the ear, facial nerve and brain
What are the causes of choleastoma
- Dysfunction in Eustacian tube
- Skin moves into middle ear
What are the classic symptoms of Choleastoma?
-Chronic otorrea
How to manage choleastoma?
mastoidectomy
What is Tinnitus
Perception of sound even when there isn’t one e.g. ringing sounds
What is the management of tinnitus?
- Treat an underlying cause if there is one
- Sound enhancement e.g. a hearing aid
- Relieve stress
What is Otosclerosis
Abnormal growth of bone in the middle ear
Presentation of otosclerosis
Conductive hearing loss
Management of otosclerosis
- Hearing enhancement
- Stapedectomy
Causes of a perforated tympanic membrane?
Infection
Grommet
Trauma
Presentation of perforated tympanic membrane
- Recurrent infection
- Hearing loss
Management of perforated tympanic membrane
- Avoid water
- Myringoplasty: closure of perforation
Causes of tinnitus
Damage to the cochlear hairs which is associated with presbycusis, noise related hearing loss and stress -
Investigation for tinnitus
- Hearing test
- If pulsatile then do a scan
Common conditions of the inner ear
- Presbycusis (old age hearing loss/starts at a frequency)
- Noise induced hearing loss (starts at a higher frequency)
- Vestibular schwannoma
What is vestibular schwannoma
A benign tumour of the vestibular nerve
what does vestibular schwannoma cause
Sensorineural hearing loss in one ear.
