Gram Negative intracellular diplococci
19y/o with septic arthritis
Neiserria (Gonnorrhoea)
Most common cause of septic monoarthritis between 19-30
Have a STI with weak treatment - then weeks later presents in joint
Gram Negative intracellular diplococci
19y/o with meningitis
Neiserria Meningitidis
6y/o child with meninitis and gram negative rods
H. Influenzae (usually unvaccinated children)
Ecoli is gram negative rods - but usually only in premature children
19 y/o student with gram + diplococci on blood culture
pyrexial and confused
Strep pneumoniae
Student where you cant culture anything, but cold agglutinins are positive
Mycoplasma
19 y/o with boil on his leg with gram +ive cocci in clusters
Staphylococcus Aureus - aggressive
(Epidermidis - less common)
19y/o with pyrexia of unkown origin
Blood cultures afeter 2 months show Gram + cocci
Strep viridans
(we all have it on our teeth - often infective after dental work eg. infective endocarditis)
Signs of Subacute endocarditis
Splenomegaly, Microscopic haematuria
Splinter Haemorrhage, Osler nodes, Janeway lesions, clubbing
eg. in older people - over months
Acute - without signs - usually IV drug users - Staph Aureus - weeks
