9 yo ate undercooked beef burgers - presents with severe abdominal cramps with grossly bloody diarrhea. No fever or fecal leukocytes. other symps: mild thrombytopenia, hemolytic anemia. Dx and treatment?
suspected: EHEC no treatment with antibiotics because more toxin if kill bacter
18 yo eval for diarrhea and abdominal discomfort. 2 days prior had intermittent, crampy periumbilical abdominal pain. PE: mild lower abdominal tenderness, fecal exam was greenish, watery stool neg for blood. Lab: feces had WBC, stool culture had slightly curved and gram-neg rods Dx? Special problems for HIV?
Campylobacter - inflammatory diarrhea HIV+ will have chronic diarrhea because can’t clear (esp when CD4
26 yo Peace Corps volunteers came back from Togo. Chronic diarrhea for 4 wks w/ crampy abdominal pain and foul-smelling flatulence soon after meals. Nausea after eating. PE: cachectic and 20 lb weight loss of 2 years. Dx? Etiology? Other infections?
Giardia hiker drinking mountain stream water, daycare malaria, typhoid, Dengue

Clostridium difficile. pseudomembranous colitis

Shigella, Salmonella, Campylobacter

Adenovirus and Rotavirus
