what is a form 7 for?
The person who feels concerned about another person’s mental health completes a form 7 and then goes before a judge who then may or may not fill out a form 8
what is a form 8 for? What is it called?
form 8 is what is filled out when the judge feels the criteria is met
-it is called a Warrent
in what other situation could a form 8 be filled?
if someone is not complying with their CTO
what is the criteria for a form 8?
A family member, caregiver, health care worker, or a peace officer will attend court if a suspected mentally ill person will NOT see a physician AND they are concerned about the person’s mental health. They must believe that the person:
1) is suffering from a mental disorder
2) likely to cause harm to that person or others OR
3) to suffer substantial mental or physical deterioration OR serious physical impairment
what is a form 10?
a police officer will complete a form 10 to apprehend and convey a pt to a designated facility
what is the criteria for a form 10?
the pt…
1) is suffering from a mental disorder, and
2) is likely to cause harm to the person or others, or suffer substantial mental for physical deterioration, or serious physical impairment, and
3) Should be examined in the interests of the persons own safety or the safety of others
how much time is given for a pt to get to the hospital in rural areas?
72 hours
what is a form 10 called?
Statement of peace officer on apprehension
what is a form 1?
an admission certificate
who completes a form 1?
a physician
what criteria must the physician feel the pt meets to be admitted in a form 1?
1) suffering from a mental disorder
2) Likely to cause harm to that person or others, OR suffer substantial mental or physical deterieration or serious physical impairment
3) unsuitable for admission to a facility other than as a formal pt
what time frame must a doctor issued a form 1 within assessing the pt?
24 hours
what happens once the pt is at the designated facility (if the first form 1 was issued in the community)
then, another doctor (different from the doc to wrote the form 1), must assess the person and they have three options
what three options does the second physician have when it comes to a form 1?
1) write another form 1 within 24 hours from the time the first form 1 was written
2) release the person
3) treat the person in the hosptial as a voluntary pt (with the persons concent)
how long does one form 1 detain a pt for?
24 hours
how long does two form 1’s detain a pt for?
one month from the date the second form 1 was written
What is a form 2 called?
a renewal certificate
What happens after a pt is detained for one month?
After one month, if the pt is still deemed to require admission as a formal pt, two physicians must separetly assess and agree that the person meets the same criteria as a form 1.
-Form 2’s have to be completed before the expiraton of the second certificate (second form 1)
