Drug Classification
Beta blocker
Mechanism of Action
β1R – antagonist →inhibits extracellular calcium entry and calcium release from sarcoplasmic reticulum in cardiac cells → negative chronotropy, inotropy, dromotropy
Pharmacokinetics
Onset of Action = Immediate
Peak Effects = 20 minutes
Duration of Effect = 13-19 hours
Half-Life = 3-4 hours
Indications
Stable Narrow Complex Tachycardia (A-Fib, A-Flutter)
Stable SVT refractory to adenosine
Contraindications
Hypotension
Decompensated cardiogenic shock
Suspected cocaine ingestion
Side Effects
Bradycardia, hypotension, lethargy, dyspnea, wheezing, dizziness
Dose
(ADULT): 5mg slow IV, IO q 5min, max total dose 15mg
(PEDI): Not recommended
Precautions
May exacerbate asthma, COPD, bronchospasm, and CHF
Drug Interactions
Do not give to patient receiving IV Ca2+channel blockers, will increase risk of profound bradycardia
Additive effects with digoxin
