What is low dose methotrexate used to treat?
Rheumatoid Athritis
Chron’s
Severe Psoriasis
When should methotrexate ideally be taken?
Once weekly
When should folic acid be taken in regards to methotrexate?
NOT on day methotrexate is taken
What side effects are we concerned about with methotrexate?
Blood Disorders - sore throat, bruising, mouth ulcers
Liver Toxicity - nausea, vomitting, dark urine, jaundice, itching
Pulmonary Toxicity - SOB, Cough
GI Toxicity - Stomatitis (sore, inflamed mouth) , Diarrhoea
How do we treat methotrexate toxicity?
Folinic Acid (calcium folinate)
Monitoring Requirements for Methotrexate
Every 1-2 Weeks then Every 2-3 Months:
FBC
Renal function
LFT
When is contraception needed in regards to methotrexate?
Use effective contraception during and 6 months after.
Women and Men !!
Drug Interactions with Methotrexate
Nephrotoxics
Antifolates (trimethoprim, phenytoin)
Hepatotoxics (rifampicin, antifungals)
Omeprazole/Esomeprazole (increases toxicity)
