What inhibits alcohol or acetaldehyde dehydrogenase? Indications for use?

A

Alcohol dehydrogenase inhibited by fomepizole
-antidote for ethylene glycol/methanol poisoning

Acetaldehyde dehydrogenase inhibited by disulfiram (Antabuse) –> accumulation of acetaldehyde leads to hangover sx–> prescribed to recovering alcoholics

Etiology and consequences of ethanol hypoglycemia?

A
  • more EtOH metabolism –> increased NADH/NAD+ ratio
  • no free NAD+ for lactate to pyruvate conversion, or G3P to DHAP conversion, or malate to oxaloacetate conversion
  • lactic acidosis, fatty liver (more G3P –> TG synthesis)
