Orientation
Orientation to person, place, time, and current situation
Affect
The observed, objective, emotional state of a person; degree of emotion or mood.
Mood
Described using the patient’s own words or summary terms; subjective
Speech
Observing the patient’s spontaneity of speech or any abnormalities
Thought Process
The quantity, tempo and form of thought.
Thought content
Evaluation of patient’s thinking(especially abnormal content)
Hallucinations (auditory, visual, tactile, olfactory etc)
Any perceived false sensory modality
Delusions
False, fixed ideas
Illusions
Distortion of real sensory modality
Judgement
Patient’s capacity to make decisions
Insight
Patient’s understanding of their illness, treatment options
Suicidal (SI) / Homicidal Ideation (HI)
Thoughts of suicide / thoughts of homicide
