When do follicles shift from AMH dependence to FSH dependence?
Starting with SECONDARY follicles the unit becomes FSH dependent - at this point its only 28 days out from ovulating
When women of reproductive age present with short menstrual cycles, what should you worry about?
Worry about Ovarian Insufficiency
What is the frequency and amplitude of LH like in the follicular and luteal phase?
Follicular:
• Low amplitude, High frequency pulses
Luteal:
• Large amplitude, Low frequency pulses
What follicle types exist within the follicular phase?
Secondary and Graafian follicles are the only ones in the follicular phase
What inhibin acts in the follicular phase?
• luteal phase?
Follicular phase - Inhibin B
Luteal phase - Inhibin A
What are the physiologic processes that lead to menses?
Corpus Luteal Regression => Decreased Estradiol and Progesterone => Loss of Endometrial Blood Supply => Sloughing of endometrial lining
What is the influence of FSH on stromal cell response to LH at the time of ovulation?
FSH acts on Granulosa cells only so, it causes these cells within the ruptured follicle to start expressing LH receptors.
When does basal body temperature physiologically rise in a female?
12-24 hours post-ovulation
When does estrogen reach its peak in the menstrual cycle?
Estrogen peaks 1-2 days BEFORE ovulation
In what phase will you see a ferning pattern of cervical mucous?
Follicular
Why might a PCOS women test positive for fertility on a fertility test?
These often look at LH to predict when ovulation will occur and patients with PCOS have a persistently elevated LH
For optimal fertility do you want FSH to be low or high?
Low FSH is key to fertility, if its high this may indicate that the ovaries are burning out
