What are the Rotterdam criteria for PCOS
Oligoamenorrhea
Hyperandrogenism (biochemically or clinically)
Ultrasound findings (*not sensitive nor specific in real life)
*obesity not a criteria, 30% can be lean
What labs do you order to help you diagnose PCOS
Testosterone - high
LH/FSH
17-OHP - rule out non-classic CAH
What are 3 things on your differential for PCOS
Non-classic CAH
Adrenal tumor
Ovarian tumor
What are the 6 broad categories for your differential for primary amenorrhea
Pregnancy
Hypothalamic
Pituitary - prolactinoma
Ovarian - primary ovarian failure, PCOS
Structural/outflow tract - imperforate hymen, transverse vaginal septum, Mullerian agenesis (Rokitansky)
Disorders of sex development
*same as secondary, but remove structural
What are hypothalamic causes of primary amenorrhea
Hypogonadotropic hypogonadism
- genetic e.g. Kallman’s syndrome
- hypothalamic - i.e. underweight/athlete triad
