MOA of caffeine:
- non-selective inhibition of adenosine receptors and phosphodiesterase
- results in the accumulation of cAMP and Ca2+
- causes smooth muscle relaxation and/or muscle/cardiac/CNS excitation
- B1 & B2 adrenergic stimulation 2* to catecholamine release
Uses of caffeine:
- cath lab (induce cardiac electrophysiological issues)
- supplement
- post dural puncture headache
MOA of octreotide:
- potent inhibitor of growth hormone, glucagon, and insulin
- decreases splanchnic blood flow
- inhibits release of serotonin, gastrin, vasoactive intestinal peptide, secretin, motilin, and pancreatic peptide
Uses of octreotide:
- acromegaly (reduce levels of GH)
- carcinoid tumor symptoms
- vasoactive intestinal peptide tumors (treat diarrhea)
Dose of octreotide:
initial dose of 50-500mcg IV
- may need subsequent doses during times of stress
MOA of flolan:
- direct vasodilation of pulmonary artery and systemic arterial vascular beds
- inhibits platelet aggregation
Uses for flolan:
primary pulmonary HTN
Flolan considerations (3):
1) abrupt withdrawal can lead to death
2) dedicated line w/ 0.22 micron filter
3) contraindicated in CHF
MOA of methylene blue:
inhibits NO-cGMP pathway:
- inhibits guanylate cyclase in the endothelium
- reduces NO relaxation of the vessels
Uses for methylene blue:
- hypotension
- hypoxia
- methemoglobinemia
Uses for indocyanine green:
- used in Neuro IR to determine if aneurysm/AVM repair was successful
Dose for indocyanine green:
0.5 mg/kg with injection at most proximal port given over 6-10 seconds
What to expect after indocyanine green administration:
transient drop in pulse O2 sats d/t spectroscopic contamination
Use for onyx glue
used in Neuro IR for embolization of AVM/aneurysms
Side effects of onyx glue use:
- severe pain at site of embolization
- breath and skin odor x 2-7 days

- oxytocin (IM)

- misoprostol (rectally/sublingual/PO)
- TXA (IV)
