Ampicillin
IV: 2 gm initial and then 1-2 gm Q4 until birth
treats Group B Strep in mother to prevent transfer to fetus during delivery
causes GI disturbance and allergic rxn
Betamethasone
IM: 12 mg Q24 hours x 2 doses
induces fetal lung maturation between 24-34 weeks gestation at risk for premature delivery
causes sodium retention and elevated BP
Calcium Gluconate
IV push: 1-2 g
treats high magnesium levels
causes N/V and constipation
Cytotec
vaginal: 1/4 of 25 mg tablet for labor
rectal: 800-1000 mcg for PP hemorrhage
induces labor or stops PP hemorrhage
causes cramping and uterine hyperstimulation
Hemabate
IM: 250 mcg, max is 2 mg
acts as prostaglandin to control bleeding after birth by stimulating smooth muscle & uterine contractions
causes H/A and N/V
Magnesium Sulfate
IV: 4-6 gm bolus then 1-3 gm p/hour x12-24 hours
acts as a preterm neuro protectant for fetus by decreasing CNS irritability/seizures and relaxes uterus to stop preterm contractions in pre-eclampsia conditions
causes decreased reflexes and hypotension
Methergine
IM: 0.25 mg
PO: 0.2 mg TID-QID up to 1 week
stimulates uterine contractions to control bleeding after birth
causes increased BP
Pitocin/Oxytocin
IV: 20-60 units in 500-1000 mL
IM: 10 units
stimulates uterus contractions for labor and given PP to firm uterus to prevent hemorrhaging
causes hyperstimulation and uterine rupture
Terbutaline (Brethine)
SQ: 0.25 mg
PO: 2.5-5 mg Q4-6 hours
relaxes uterus in preterm labor or with a tetanic uterine contraction during induction/fetal distress
causes tachycardia in mother/fetus and is uncomfortable
Duramorph (Morphine)
SC/IV/IM/epidural/intrathecal: 10 mg every 4 hours PRN initially
given as epidural narcotic analgesic for pain relief without loss of motor/sensory/sympathetic function
causes sedation and constipation
Naloxone (Narcan)
SC/IM: 0.4 mg Q2-3 mins PRN
treats opioid overdose by occupying opiate receptors
causes tachycardia and N/V
Fentanyl
IV: 100 mg
treats labor pains by increasing pain threshold and altering pain perception
causes apnea and respiratory depression
Ketoralac
PO: 10 mg Q6 hours
IV: 30 mg
IM/IV: Q6 hours
treats PP pain by blocking COX enzymes that are needed to produce prostaglandin
causes nausea and abdominal pain
Nitrous Oxide
intermittent inhalation: 50% O2 and 50% N2O
used as labor analgesia
causes nausea/vertigo and euphoria
Nalbuphine/Nubain
IM/IV: 10-20 mg/70 kg Q3-6 hours PRN
used as labor pain opioid by altering response to pain
causes sedation and respiratory depression
