Adrenaline
Anaphylaxis, severe asthma, cardiac arrest and ROSC
No contraindications
Intramuscular 0.5ml drawn from a 1:1000 vial
Intravenous 1ml drawn a 1:1000 vial (only cardiac)
Repeated every 3-5 minutes no max
Amiodarone
Cardiac arrest post three consecutive shocks (VF/VT)
No contraindications in this use
Intravenous 300mg in 6 ml drawn from 2 vials of 150mg in 3 ml
Repeated 150mg in 3 ml dosage after another three consecutive shocks
Aspirin
Acute coronary syndrome (cardiac chest pain)
Hypersensitivity to salicylates, active bleeding or bleeding tendencies (clotting disorders)
Oral chewed 300mg tablet single dose
Cophenylcaine
Epistaxis and oral haemorrhage
Hypersensitivity to amide anaesthetics and pregnancy
Intranasal 5mg in one spray
Per oral 5 mg one spray
Repeat 1-2 minutes max of 25 mg or 5 sprays
Fentanyl
Significant pain
Hypersensitivity to fentanyl or other phenylpiperdines
Intramuscular 25mcg in 0.5ml straight from 100mcg in 2ml vial
Intravenous 25mcg in 2.5ml as whole 2ml vial drawn and diluted with 8 ml saline
Intranasal 60mcg in 0.2ml from 600mcg in 2ml drawn extra 0.1 to prime aspirator
Gastrolyte
Dehydration
No contraindications
Per oral one sachet in water
Repeat as needed
Glucagon
Hypoglycaemia 2nd line
Hypersensitivity to glucagon/lactose and pheochromocytoma
Intramuscular 1mg via giving set
Glucose 10%
Hypoglycaemia 1st line in unconscious or risk of aspiration
No contraindications in this setting
Intravenous 10g in 100ml from 50g in 500ml draw 5X 20ml vials with a bag of saline drip FLUSH after ever syringe
Repeat in 5 minutes if needed
Glucose gel
Hypoglycaemia 1st line if alert
Altered GCS and paediatric under 2
Per oral 15 g sachet
Repeat as needed
Glyceryl trinitrate (GTN)
Acute coronary syndrome (cardiac chest pain) acute cardiac pulmonary oedema
Hypotension under 90 systolic, symptomatic Brady/tachycardia and use of erectile dysfunction meds in 24 hours
Sublingual 0.4mg one spray
Repeated every 5 minutes no max
Hydrocortisone
Anaphylaxis, severe asthma, exacerbation COPD
hypersensitivity
Intramuscular 200mg in 4 ml 2X vials
Intravenous 200mg in 10 ml dilute with 6 ml saline
Single dose
Ipratropium bromide
Severe asthma and COPD
hypersensitivity to ipratropium bromide and atropine
Nebulise 500mcg in 2ml 2X vials WITH 5mg in 2ml salbutamol
Repeat as needed
Methoxyflurane
Mild/moderate pain
Malignant hyperthermia, eclampsia, use of tetracycline antibiotics, hyperthermia, liver/ renal diseases, altered GCS and behavioural disturbances
Inhaled 3ml vial whistle
Single repeat after 20 minutes
Midazolam
Seizure and sedation
Hypersensitivity to benzodiazepines, symptomatic hypotension and shock
Intranasal 5mg in 1ml
Intramuscular 5mg in 1ml
Intravenous 2.5mg in 2.5ml dilute vial with 12ml saline making 15mg in 15ml
Repeat after 5 minutes max 15 mg
Morphine
Severe pain
Active labour, symptomatic hypotension and hypersensitivity to opiates
Intramuscular 5mg in 0.5ml
Intravenous 5mg in 5 ml dilute with 9 mls saline
Repeat after 5 minutes max 20mg
